It is somewhat remarkable to read about management culture. I mean, I have seen and dealt with incompetence on all levels up the chain of command but there never seems to be a reason to become part of the problem. Yet on so many cases whether it is project or LOB management so often irrational decisions are made that can't be explained or make sense. There must be a common fault when the "manager' bit flips on in peoples head that make them go out-of-bounds.
