@Martin somwhat related. What you need to do here first is "nomalize" the different ways a country is spelled in location to a single value for that country, then do the top as offered in the other examples.
the normalize part is unique here

I have edited the query for Slovakia to remove unnecessary "lower". (This is the recent revision.)
I might get back to that query when I have more time.

Only after your ping I have noticed that the part with exclusions was still case sensitive.
I do not think there is something impressive there - for any country one could go on and on, adding further cities, towns, villages, districts.
One thing I should keep in mind: When I edit a query and I want to test it, it is better to run it on a smaller site (such as MO) than on a big site (like SO).
 
Yeah, I have this idea to move some of that info in a table. I'll use your query as inspiration and see if I can come-up with something that is generalized and still able to meet your functionality.
