@Flyk we finally got a staging server last month. It used to be local dev -> production.

I guess the answer to your question is: people who don't want to spend money :)

@stijin @Flyk - I saw environment when the steps to switch from the SharePoint test environment and the production one were "Ok, add the domain users as viewers to the site coll, please"

we're working hard on replacing Orion internally, that'll be open sourced as soon as it's solid and a bit more polished as well, that'll make Opserver & it a completely free replacement for most people's needs
